The lore, the fanart etc. 

Like kantai collection in some ways.  I played perfect cherry blossom in 2004.  I don't even care about Bullet hell anymore.  Just like the stories, art, animations etc.

I don't play Touhou, but the soundtracks are killer. And there are lots of cool fan arrangements of the music as well.

I think that the games and media associated with it look ugly and chaotic, but that's just me. I don't really know, this stuff just feels not neat and not pretty, low effort and crazy.

Great games with a lot of depth and challenge, great aesthetics and music, and it's basically a cute revival of Japanese and East Asian mythology.
It has a very productive and creative fanbase, and also has a great legacy online if you care about that.

All of this was made by just one man, so of course he can't do literally everything perfectly. But even then, you don't pay as much attention when you're actually playing the games because they actually require you to be focused instead of being baby movie game bullshit. And even then, there's beauty in the bullet patterns, etc.

For a long time, I used to think it was an anime, didn't even know it was a game. And even then I'd have imagined it to be an RPG.

Try actually playing some of the games.  Their main strength, aside from the music, is that the attack patterns and their theming when it comes to bosses can be genuinely interesting compared to a lot of other schmups.

I also think what makes it so appealing, is how doujins have their own interpretation of the world (like putting in vehicles and full on cities) and give 2hu girls different types of personality.

The fanmade music and fanmade games had me interested years ago. Mostly I liked the progressive remixes on jewtube of which at one point were all removed over copyright infringing.
