There are ways to tell the difference. For instance, you can see the thin, single strands of hair around the shoulder, neck, and forehead in the pic. From what I've seen, AI has not been able to replicate this, usually going with just multiple strands narrowing down to one point as it's more common in anime hairstyles.
Also, the shadows are more accurate in the pic than what AI would produce. It has gotten better at it, but you can still notice the odd lighting or shadowing on parts that shouldn't have them in ai-gen art.
There are also moles around the shoulder and groin. AI-gen will normally make the skin look as flawless as possible, to the point of looking shiny/glossy as others have mentioned. AI-gen will either try to make the art look more realistic or like an obvious heavily-photoshopped pic (though some have tried to generate pics more hand-drawn/colored). You won't see much ai-gen art that shows flaws/unique traits that add more to the character portrayed. Usually any flaws are things really out of place or the AI shitting itself on what the person giving the data meant (such as "character eating ramen with hands")
In addition to seeing strokes and lines that AI would remove, I have yet to see ai-gen draw pubic hair well in general. Perhaps it's due to bare/shaved being more popular in hentai, but not sure.
Lastly, if one's natural instincts are not completely numb or gone, humans can naturally tell when something is wrong, stands out, or just off in a pic or drawing, similar to a gut feeling about a situation going bad.
Given the rate AI is improving, it will probably get harder to tell, but there will always be signs showing what was hand-drawn, and what is generated.
